out IACUC forms are located to the left. The AWIC Alternative
Brochure and the NIH Alternative Search Tip Sheet provide information
concerning the welfare of research animals.
All research
conducted at this institution must undergo an IBC Review. IACUC approval
will not be given until IBC exemption or approval is received. A link to the Institutional Bio-Safety Committee (IBC)
webpage is also
located on the left.
Research
Protocol Application Form

Before entering
information into a form, you must
first download the form to your computer. Use FILE, SAVE AS, select
your directory (SAVE IN) and create your own FILE NAME.

Close this website then
open the form that you have saved on your hard drive or server.

Complete each item on the form or indicate
“Not Applicable.”

Please email the forms and supporting
documents as directed on the application with
IACUC Protocol Review, IACUC Breeding Colony Review or IACUC Amendment in the
subject line.

Sign and send the signature page along
with supporting documents not available in an electronic format to the
IACUC Office, Resource Center, 433 Bolivar Street, Room 206.

The deadline for
receipt of applications in the IACUC Office is the last Monday of the
month. IACUC meetings are held on the third Monday of every
month.

Use the Arrow Keys, Tab Key or click in
the grey shaded areas to complete the form. To “check” a box, click on
the box.

Hitting “Control + Tab” will insert tabs
within a shaded area.

You cannot spell check these fillable Word forms. If you
want to spell check long answers, enter the information in a word
processing document, spell check the information then copy and paste it
into the shaded area.

Because of formatting issues do not paste
tables or spreadsheets into shaded areas. Email them as separate
documents.

To access web or email addresses within
the form, use the
arrow key or tab key to select the shaded area containing the address, right
click, select COPY, and then paste the address into your web browser or
email program.
