Temporary Location
We are temporarily located in the Library on the 4th floor of the Resource Center Building at 433 Bolivar Street in New Orleans. In order to reach our office, it is necessary to take the elevator to the third floor and use the Library's internal elevator (or stairs) to access the fourth floor.

The Office of the Registrar is
committed to providing faculty, staff, and students an array of
exceptional academic services. The staff of the office works constantly
to improve and enhance the quality of these services to benefit the LSU
Health Sciences Center New Orleans community. The office welcomes your
comments and suggestions to help us
better understand and serve your needs.
Our office is located on the fourth
floor of the Library, Administration, and Resource Center Building (RCB)
at 433 Bolivar Street. Regular office hours are from 9:00 a.m. to
4:00 p.m. Monday through Friday

It is the responsibility of each student to read, understand and apprise themselves of all the terms, conditions, and regulations set forth in the current LSU Health Sciences Center New Orleans Academic Catalog / Bulletin. Degree Verification
LSUHSC New Orleans has authorized the
National Student Clearinghouse to act as our agent for verification of student degrees. Outside agencies requiring a verification of a degree should be directed to contact the Clearinghouse online at http://www.degreeverify.com or by phone at (703) 742-4200.
Enrollment
Verification
Enrollment Verification Certificates are
available to currently enrolled students. Students may print their own
verification certificate by using our secure
Student Self-Service Portal provided by
the National Student Clearinghouse. Outside
agencies requiring a verification of enrollment should
contact the Clearinghouse online at
www.studentclearinghouse.org or by phone at (703) 742-4200.
Student Self-Service
Student Self-Service allows currently enrolled students to print
enrollment verification certificates, order transcripts, view enrollment
history and enrollment verification provided at their request, check
loan deferments sent to lenders, and link to real-time information on
their student loans. Students can access Student Self-Service by
logging onto our
Student Self-Service Portal using their
LSUHSC user id and password.
Student Forms
Application for Resident
Classification - Students must complete
and submit this form, along with copies of any supporting documentation,
to the Office of the Registrar in order to be reclassified as a
Louisiana resident. The application must be submitted
no later than 21 calendar days
following the first day of classes of the term for which such
reclassification is sought. LSUHSC's programs in Medicine,
Dentistry, and Nurse Anesthesia consist
of a single annual term in each academic year; therefore, all applications
for resident classification by students enrolled in these programs must
be submitted at the beginning of the academic year, usually in August. LSU System residence
regulations are described in
PM-31.
Change of Address -
Changes of address may be made by completing this form and sending it to
our office. In addition, address and telephone number changes may
be made by emailing us at
registrar@lsuhsc.edu from your LSUHSC email account.
Request for Duplicate Diploma -
A replacement diploma may be ordered by completing this form and
sending it to our office, along with the appropriate payment as
indicated on the form.
Transcript Request -
LSUHSC transcripts may be ordered and paid for online at
www.getmytranscript.com.
Individuals who are unable to use the online service may submit their
requests by completing this transcript request form and sending it to our
office, along with the appropriate payment as indicated on the form.
Administrative Forms
Certificate Order Form - This
Excel spreadsheet should be completed and emailed to
registrar@lsuhsc.edu whenever
residency certificates are needed. Normal processing time for
certificates is 4 to 6 weeks.
Change in
Academic Status - This electronic form
is to be used by the Offices of Student Affairs to notify student
records officials of any changes in a students academic status.
Termination of
Enrollment - This electronic form is to
be used by the Offices of Student Affairs to notify student records
officials whenever a student has notified them of their intent to
withdraw or take a leave of absence from an academic program.
Request to Add/Change Curriculum - This
paper form
is to be used by academic program directors to add a course to the
curriculum for a particular program, or to change the existing
curriculum.
Request for New Course -
This paper form
is to be used by academic program directors to add a new course to a
schools course inventory.
