2022 Faculty Evaluation Process - General Guidelines

LSU Health Sciences Center - New Orleans is committed to a culture of employee success, continuous improvement, and ongoing performance feedback.  A critical component of these goals is the completion of our annual performance evaluation and success planning process.  

The following general guidelines apply specifically to the Faculty Evaluation Processes in place across campus. *Please check with your Department  Leadership or Business Managers regarding the 2023 Faculty Evaluation Process in place for your School.

The Faculty Supplemental Rating Form has been updated to include the Self-Evaluation component. If the employee has opted to complete a Self-Evaluation, the Self-Evaluation form MUST be submitted alongside the evaluator's form.

 

2022 FACULTY EVALUATION GENERAL GUIDELINES 

  • Faculty have been approved to use the existing Faculty performance evaluation form within their specific school/divisions, so long as there is an overall numeric rating provided. Some Schools will be using the Faculty Supplemental Rating Form linked below.
     
  • School of Medicine
    • SOM encourages both the SOM Annual Faculty Review Form and the Faculty Evaluation Supplemental Rating form, though only the supplemental rating form is required for merit purposes.
       
  • School of Dentistry
    • SOD will use both the SOD Annual Faculty Review Form and the Faculty Evaluation Supplemental Rating Form.
       
  • School of Public Health
    • SPH will use the SPH Annual Faculty Review Form.
       
  • School of Allied Health Professions
    • SAH will use a revised SAH Annual Faculty Review Form that incorporates an overall rating scale.
  • School of Nursing
    • SON will use the existing SON Annual Faculty Review Form.

Submission Process:

  • The same submission process is in place for Unclassified Staff and Faculty Evaluations for 2023.
     
  • The HRM Evaluation Submission Portal should be used to submit completed Faculty Evaluations to HRM upon completion.  Faculty should send their individual self-evaluations to their evaluating director or supervisor.
     
  • This will support reporting related to the 2022 Merit Process.
