Application for LSU Health Foundation Campus Disaster Relief

LSU Health New Orleans employees and students in extreme financial need due to the tornadoes and severe storms on Wednesday, December 14, 2022, may apply for emergency financial assistance from the LSU Health Foundation Tornado Relief Fund. Many employees and students sadly face difficult hardships, but because of the limited monies available in the Fund, only those who have suffered financial hardship due to the tornadoes and severe storms on Wednesday, December 14, 2022, are eligible.

Employees and Students Eligible for Assistance

All current employees and currently enrolled students in good standing are eligible. If several family members work or attend LSU Health New Orleans, only one will be eligible to receive monies from this Fund on behalf of the family.

A Family Member is defined as the employee’s or the student’s spouse, partner, children, or those family members for whom the employee or students are financially responsible.

Eligibility is determined without regard to race, color, religion, sex, national origin, age, disability, military status, or other protected class.

Expenses Qualifying for Possible Reimbursement following the tornadoes and severe storms on Wednesday, December 14, 2022

Merely suffering through a tornado or severe storm does not mean reimbursement will occur. Losses or expenses covered by insurance or FEMA assistance will not be considered appropriate for reimbursement from the LSU Health Foundation Tornado Relief Fund. However, expenses due to losses not covered by insurance or FEMA assistance will be considered.

Applicants must verify they do not have access to other financial resources. To be considered, the application must be fully completed before or on February 15, 2023.  Incomplete applications will not be considered.

Any assistance you receive may be subject to income tax; a W-9 for IRS purposes might be requested from you. If you have questions, you should seek the advice of a qualified tax preparer.
