Public Records Request

LSUHSC-NO is committed to providing access to public records in accordance with Louisiana Public Records Laws.

LSUHSC-NO will make all reasonable effort to respond to requests for access to public records as soon as possible and in accordance with the provisions of the Public Records Law.  However, large requests will take longer to collect and process because all requests must be reviewed for exceptions to the Public Records Law by LSUHSC-NO before they can be inspected, copied, and/or reproduced.  Notice will be provided to you in all instances where the public nature of the records is in question.

  1. Name, mailing address and telephone number of the requestor;

  2. Identify the record(s) being requested with reasonable particularity; and

  3. Indicate whether inspection or which method of receipt is desired.
