We value your voice and encourage you to report any concerns you may have so we can respond effectively, identify trends, and determine appropriate solutions and resources.

Anonymous reports should be submitted through the LSU Ethics, Integrity, and Misconduct Helpline (EthicsPoint).
Please note that anonymous reports may limit our ability to investigate and take appropriate action.

LSUHSC-NO strictly prohibits retaliation against any Individual who reports concerns, files grievances, and/or participates as a witness or involved party. Anyone who believes that have experienced retaliation should immediately report it to the Office of Student Advocacy & Title IX at (504) 568-2211 or lsmi30@lsuhsc.edu.

 

Please review the forms below and select the one that most accurately describes your concern. 
Each dropdown provides the purpose of the form, the office that will receive the form, and a link to submit the report.

The form is used to report disruptive or concerning behaviors; difficulty accessing resources; academic distress; personal, emotional, and/or psychological difficulties; or potential harm to self or others. This would include behaviors or concerns by any LSUHSC faculty, staff, or student. 

This form will be sent to the Behavior Intervention Team (BIT). 

Submit a Concerning Behavior Report. 

Anonymous reports of any kind should be submitted through the LSU Ethics, Integrity, and Misconduct Helpline (EthicsPoint). 

Please note that anonymous reports may limit our ability to investigate and take appropriate action.

Submit an anonymous report through EthicsPoint. 

This form is to be used to report student-related incidents pertaining to academic tasks such as cheating, plagiarism, falsification, etc, as well as unprofessionalism.  It is also used to report any complaints of an academic nature or connected to an academic setting such as a classroom, laboratory, clinic, etc.

This form will be sent to the Academic Affairs or Student Affairs Office of the applicable school. 

Submit an Academic Misconduct or Complaint Incident Report.

This form is to be used to report any behavioral misconduct of students that would include but is not limited to violations such as physical violence/assault, drugs/alcohol, weapons, theft, etc.

This form will be sent to the Office of Student Advocacy & Title IX. 

Submit a Nonacademic Misconduct Incident Report. 

This form is to be used to report any conduct that discriminates, stereotypes, marginalizes, excludes, harasses, or harms anyone in our community based on their identity or protected civil rights. This is to be used for conduct based on race, color, ethnicity, national origin, disability, age, religion, or political ideas, affiliation, or ideology. This would include behavior by any LSUHSC faculty, staff, or student. 

Conduct based on sex, gender, gender identity, or sexual orientation should be reported through the Title IX & Power-Based Violence Incident Report Form. 

This form will be sent to the Office of Diversity and Inclusion. 

Submit a Bias or Discrimination Incident Report. 

This form should be used to report incidents of sex-based discrimination, sex-based harassment, sexual misconduct, and power-based violence or mistreatment. This includes conduct based on sex, gender, gender identity, sexual orientation, and pregnant/parenting status. This would include behavior by any members of the University community, including faculty, staff, students, volunteers, organizations, and any other affiliate that participates in activities associated with LSUHSC. 

This form will be sent to the Title IX Coordinator. 

Submit a Title IX or Power-Based Violence Incident Report. 

This form should only be completed by the individual who experienced the unwanted Title IX/power-based violence behavior. This individual is considered the Complainant under Title IX and is the only person who can file a Formal Complaint with LSUHSC.

This form will be sent to the Title IX Coordinator. 

Submit a Title IX or Power-Based Violence Formal Complaint. 

To report all issues of suspected Fraud, Waste or Abuse of Assets, Misappropriation, Ethics Violations, Conflicts of Interest, and violations of LSUHSC policies. 

This will be sent through the LSU Ethics, Integrity and Misconduct Helpline (EthicsPoint). 

Submit an Ethics Violation Report. 

This form is to be used when reporting general concerns or complaints about offices, departments, etc or the general campus climate.  It may also be used if any of the other forms do not appear applicable to your concern or if you are unsure which form to select. 

This form will be sent to the Office of Student Advocacy & Title IX. 

Submit a General Concern Report. 

Grievances or complaints between between employees that does not apply to any of the other report forms or any other employment related concerns may be submitted through LSU Ethics, Integrity and Misconduct Helpline (EthicsPoint) or directly to Human Resources Management, Employee Relations. 

Submit an Employee Relations grievance or complaint through EthicsPoint. 

Submit an email directly to the Human Resources Management, Employee Relations Manager.
