Change of Grade Process
If an instructor needs to change a grade after final grades have been processed, they must complete a Change of Grade Request form. Instructors can obtain the Change of Grade Form from the Registrar's Office. Grades may be changed by the instructor who is responsible for submitting the grades for the course, and for valid reasons only. Instructors should submit the completed and signed forms to the Registrar's Office to be processed. The signatures of the instructor, course director, department head, and dean are required. The instructor will be notified when the grade has been changed in the system. If the instructor does not receive notification within a few business days, they will need to follow up with the Registrar's Office at registrar@lsuhsc.edu.
Grade Systems
All schools in the LSU Health Sciences Center - New Orleans use a letter grading system (A, B, C, D, F, P, S, and U) with the exception of the School of Medicine. Official final grades for the School of Medicine are recorded as Honors (H), High-Pass (HP), Pass (P), Fail (F) and Withdrew (W).
Incomplete Grades (I)
A grade of Incomplete is assigned when the student has been unable to complete the course requirements in the usual time or manner. A Change of Grade Form must be submitted to the Office of the Registrar within 14 calendar days of the beginning of the following term. On completion of the required course work, a Change of Grade form must be completed then forwarded to the Office of the Registrar. The request will be evaluated and processed. Then the grade of Incomplete will be changed to the appropriate final grade on the official record. If the form is not submitted, the "I" grade will be converted to an "F."
