Changing Your Legal Name
The Office of the Registrar manages name changes in the University's student directory. Currently enrolled students who need to change their legal name must submit their request to the Registrar's Office. To start the request, complete and sign a Request for Legal Name Change Form. Supporting documents are required to process your name change request. Supporting documents include:
- Certified court order, or dissolution decree reflecting the new name in full
- Current passport or official proof or identity, certified by U.S. embassy abroad or by the approriate foreign embassy in the U.S.
- Marriage License AND your social security card or valid driver's license.
Send the form along with supporting documents to registrar@lsuhsc.edu. Name changes are completed in 3 to 5 business days and will reflect in Student Self Service. Pleae note: Your @lsuhsc.edu email user name will not change; however, your updated name will populate in the directory.
Preferred Name and Pronoun Policy
A preferred name and pronoun will only be available for ID badges. Preferred names will not apply to LSUHSC email addresses, transcripts, enrollment verifications, or diplomas. Preferred names and pronouns will be displayed on the front of student ID badges. Legal names, student ID numbers, and signatures wil be visible on the back of the ID badge. Currently enrolled students who wish to have their preferred name and pronoun displayed on their student ID badge must complete the Preferred Name and Pronoun ID Badge Form. Submit the form along with your current ID badge to the office in person.
Office hours are Monday through Friday, 8 AM to 4 PM.
