INCIDENTS AND NON-COMPLIANCE


All members of the LSUHSC community are obligated to report, or assure reporting of, their knowledge of any event (incident) that results in comprised biosafety resulting in personal injury or a "near miss" that could have resulted in personal injury due to comprised biosafety. Incidents may include adverse events, unanticipated problems, adverse animal exposure, deviations from approved protocols and instances of suspected non-compliance with laws, rules, regulations and policies.

Reporting Incidents & Non-Compliance

Knowledge of incidents may be reported directly to one or more of the following parties. The LSUHSC-NO hot line and U.S. mail may be used to submit information anonymously.
