2024 Insurance ID Cards
The medical, dental, and vision plans will not be mailing out new cards for 2024 as there were no plan changes for continuous enrollees. However, if you enrolled in any of these plans for the first time or switched to a new plan, you should be receiving new cards. In the event they are not received timely, please reach out to the insurance companies directly and/or create online accounts to access member information and digital ID cards.
Need to update your address? Click here! Active LSUHSC-NO employees can now make personal data changes electronically. From the Main Menu, navigate to Self Service – My Forms and select Personal Data Change Form.
