Registration forms are required every academic year. They are due before the start of every Fall Semester and must be submitted prior to the start of classes.
Completed registration forms can be emailed to the Bursar's Office at firstname.lastname@example.org.
There are three (3) required forms:
1. Optional Fee Checklist
2. Student Accident & Sickness Plan
3. Authorization for Use of Title IV Funds
NOTE: Student health insurance premium and/or needle stick fees cannot be paid with financial aid (even with Title IV authorization).
